Transition planning is a process used to assist students with disabilities to move from school life to adult life. It's a cooperative effort between the school, the student, the family and community agencies. South Carolina public schools require that the transition process begin at age 13, but it's never too soon to start planning for the future.

Planning your child's future should be based on what they need, what they are good at, what they like, and what they want to do [IDEA § 300.43]. The whole process may seem overwhelming, but luckily there are many resources available to help. Take a look at some of the resources we've compiled below.
